Dementia Well Pathway: Challenges and Opportunities for Personalised Care

A joint session between Alzheimer’s Society and DCAN to present the findings of dementia pathways research and explore potential impact comprehensive model of personalised care can have.

Alzheimer’s Society have recently published a major report – From diagnosis to end of life: the lived experiences of dementia care and support.

This report looks at four stages of NHS England’s Well Pathway for Dementia:

Diagnosing Well
Supporting Well
Living Well
& Dying Well.

It explores in detail what NICE and the Government say people in England should be receiving at each stage and benchmarks it against the experiences of people affected by dementia.

Dementia Change Action Network (DCAN) is hosting a webinar with the report’s authors, Ella Robinson and Kielan Arblaster from Alzheimer’s Society’s Policy team. You will receive an overview of the report and its key findings, as well as an update on what response the report has had so far.

You will also hear details of our plans within DCAN to ensure Universal Personalised Care becomes a key driver for positive change, and ways you can get involved.
