This new Special Report from the Coalition for Personalised Care is focussed on highlighting the importance of community strengthening roles, including how individuals and communities can best take control of their own lives and environment to maintain good health and well-being.

The report explores a place-based approach in York and Wakefield and examines how best to align link worker and capacity building activities with optimum success.

It highlights best practice and offers helpful tools and guidance to health and local authority colleagues who are keen to find ways to collaborate across sectors. The report was commissioned by NHS England and delivered by C4PC in collaboration with The National Association of Link Workers (NALW), the New NHS Alliance (now The Health Creation Alliance) and the Social Care Institute for Excellence (SCIE).

The report will be shared and discussed via a webinar on Thursday 18th March from 10am  to 11.30am which will offer the opportunity to hear more detail about the findings as well as the chance to put questions to an expert panel. A number of high profile speakers will take part in the event.
